By enrolling in our Direct Primary Care Membership, you acknowledge and agree to the following:

• No Long-Term Commitment: You may cancel your membership at any time with a minimum of 30 days' written notice.

• Ongoing Billing: If cancellation is not submitted with proper notice, membership billing will continue on a monthly basis until notice is received.

• Billing & Payments: Membership fees are non-refundable and billed monthly.

• Invoice Notification: An invoice will be sent to you on the date this form is signed, and payment will be due as indicated.
