Membership Policy
By enrolling in our Direct Primary Care Membership, you acknowledge and agree to the following:

No Long-Term Commitment: You may cancel your membership at any time with a minimum of 30 days' written notice.

Ongoing Billing: If cancellation is not submitted with proper notice, membership billing will continue on a monthly basis until notice is received.

Billing & Payments: Membership fees are non-refundable and billed monthly.

Invoice Notification: An invoice will be sent to you on the date this form is signed, and payment will be due as indicated.

By proceeding with enrollment, you confirm that you have read, understood, and accepted these terms.
