By using the services provided by Direct Access Primary Care, you agree to the following terms and conditions:

1. General Terms

• Direct Access Primary Care operates on a membership-based and direct-pay model, meaning we do not bill insurance for primary care services.

• Our services include medical consultations, assessments, procedures, medical weight loss, and wound care. Certain services such as medications, bloodwork, hospital imaging, immunizations, and cosmetic treatments (e.g., Botox, fillers) are not included in membership pricing and will require additional payment.

2. Appointments & Booking

• All appointments must be scheduled in advance.

• Once a booking is confirmed, refunds are not available. However, a one-time reschedule option is offered if you notify us at least 24 hours in advance.

• Late arrivals exceeding 15 minutes may require rescheduling based on provider availability.

• No-shows are not eligible for refunds or rescheduling.

3. Membership & Payments

• Monthly membership fees are due on the scheduled billing date. Late payments may result in service interruptions.

• Membership services are non-transferable and can only be used by the registered patient.

• Failure to make timely payments may result in membership cancellation.

4. Cancellation Policy

• Memberships can be canceled with a 30-day written notice.

• Any unused services or prepaid fees are non-refundable.

5. Medical Disclaimer

• Direct Access Primary Care provides non-emergency primary care services. In case of an emergency, patients should call 911 or visit the nearest emergency room.

• Our services do not replace the need for hospital care, specialist referrals, or urgent medical intervention.

6. Privacy & Confidentiality

• We are committed to protecting your medical records and personal information in compliance with HIPAA regulations.

• Patient information will not be shared with third parties without consent, except when required by law.

7. Modifications to Terms

• Direct Access Primary Care reserves the right to update these Terms & Conditions at any time. Patients will be notified of significant changes affecting their membership or services.
